We use the proven Pathways System with Select Rehabilitation’s program. This approach combines Pathways with our GEMS program, both built on the Allen Cognitive Level system and Teepa Snow’s GEM approach. Our simple color system – green, yellow, and red – helps us know where each resident is so we can provide them personalized attention and care.
Our certified dementia practitioners work with therapists to create special care plans for each person. We explore the best ways to support and compensate for any skills they may have lost. Each resident receives care tailored to their exact needs, not a one-size-fits-all plan.
