• This week, several states have started to relax restrictions from previous stay at home orders given by the Governors of each state regarding the COVID-19 pandemic. This is a notification to inform you that even with the state’s re-opening plan, long-term care facilities are not covered in the first scheduled phases. For Colorado, state is preparing to begin opening select businesses in a phased approach, beginning with the Safer at Home phase which keeps many of the same recommendations as the initial Order. You can view this plan here. Separate from this plan, there is still a current, Public Health Order 20-20 that restricts all visitors to all Colorado nursing facilities, assisted living residences and intermediate care facilities. This was last amended in April 20th and can be viewed here on Colorado’s Official State Web Portal. In addition to the Governor’s report, The White House Administration recommends that visits to senior care facilities or hospitals should be prohibited until Phase Three of the nation’s opening approach. Therefore, we are reminding you that we are still prohibiting non-essential visitors to our communities until further notice. • We continue to follow guidelines from the Centers for Disease Control and Prevention, the Centers for Medicare and Medicaid Services and local health departments. In doing so, we are currently restricting all visitors and non-essential healthcare personnel to our community until further notice. Essential healthcare personnel include, but are not limited to, Physicians, Nurse Practitioners, Home Health, and Hospice Providers. There are exceptions to this for end-of-life care or other situations pursuant to the guidance of public health officials. All healthcare visitors are subject to health screenings prior to being allowed inside the community. Additionally, group activities and community events have been cancelled until further notice and our Food and Beverage department is working to deliver meals to residents in the comfort of their apartments, all steps we are taking to practice social distancing within our community. It is essential that every resident feels connected daily during this time. We have implemented resident engagement programs to keep residents’ spirits up and keep them active and engaged with each other while still practicing our social distancing. We have established daily FaceTime calls to loved ones, activities that residents can participate in either from their apartment or apartment entryway inside the hallways, and our My Special Friend program in which each Assisted Living and Memory Care resident is paired with a community staff member that can provide assistance to the resident, do activities with the resident, and answer questions for the resident. On Wednesday, the World Health Organization (WHO) officially declared Coronavirus a pandemic. During the President’s Coronavirus Address on 3.11.2020, he stated that “…we are strongly advising that all nursing homes for the elderly suspend all medically unnecessary visits.” Although our communities are not categorized as nursing homes, they are considered long-term care facilities for the elderly, which is the most at-risk demographic for this virus. Therefore, due to the continued pattern of transmission of COVID-19 in the United States and the evidence of community spread, our Company has made the tough decision to stop allowing non-medical visitors into our communities nationwide. We apologize for the frequent changes in the visitation process, but as you are aware, this is a rapidly evolving situation. We will assist residents in FaceTime visits with family members and other engagement activities. Physicians, Nurse Practitioners, Home Health, and Hospice Providers will still be granted access, and will undergo a travel screening, symptoms screening, and have their temperature checked prior to being allowed inside the community. If there are extenuating circumstances in which a family member needs to visit a loved one, we are asking them to please contact the Executive Director of the community.

Additionally, all community events have been cancelled for all communities nationwide for March and April.

We do not have any suspected or confirmed cases of COVID-19 in any of our communities, but feel these additional precautions are necessary to protect our residents and employees.

• At this time, our community has not had any suspected or diagnosed cases of the Coronavirus. However, due to the fact that our residents are an at-risk community, beginning on March 7th, our community has determined to follow the recommendations of local, state, and federal government officials to halt allowing non-essential visitors into our community for a period of one week. All healthcare and medical agencies, employees, and vendors will continue to be allowed into the community during this time. All healthcare personnel, please note that if you need to visit the community during this time, you must contact the community first to make an appointment. Please be advised that upon arrival at the community, you will be asked to complete a screening process that will include a temperature check. Thank you in advance for your cooperation as we work to protect the safety of our residents and employees.

In the Report to Governor Desantis from The Re-Open Florida Task Force on the Safe.Smart.Step-by-Step. plan to re-open Florida, which is attached to this letter, the Task Force indicates on page 14 that visits to long-term care facilities remain prohibited and that “…these restrictions may be lifted by order of the Governor in consultation with state health officials and should not be a component of the phased-in the Safe. Smart. Step-by-Step. plan.” At this time, the Governor has not lifted this restriction for long-term care facilities. In addition to the Governor’s report, The White House Administration recommends that visits to senior care facilities or hospitals should be prohibited until Phase Three of the nation’s opening approach. For Kentucky, the Governor will be implementing the first phase of opening businesses for the State under the Healthy at Work plan phased approach that will allow certain businesses to open at certain capacity thresholds. During Phase 1 of Healthy at Work, the Kentucky Department for Public Health will determine whether Kentucky has met certain public health benchmarks for reopening Kentucky’s economy. These benchmarks closely follow the White House’s Guidelines for Reopening America. During Phase 2 of Healthy at Work, expected to begin in June, the Department for Public Health will evaluate individual businesses’ ability to safely reopen. The evaluation of business being able to open will be determined by certain public health benchmarks. Currently, the number of infections of staff and residents in nursing and long-term care facilities in Kentucky continues to grow. These types of facilities are excluded from phase 1 and 2 of the Healthy at Work plan and remain under the same restrictions as put forth in the Governor’s mandate for state run facilities and guidance for operators on March 10th to continue to prohibit non-essential visitors to facilities. In addition to the Governor’s report, The White House Administration recommends that visits to senior care facilities or hospitals should be prohibited until Phase Three of the nation’s opening approach. OURS offers a phased approach to reopening Oklahoma’s economy and is based on criteria set by public health experts. Phase 1 and Phase 2 of the plan are expected to continue through May, and both recommend that visits to Senior Care Facilities still be prohibited. Once hospital and incident rates remain at a manageable level statewide for 14 more days following Phase 2, Phase 3 will apply, which will allow for further public interaction. This Phase of the State’s plan, however, continues to recommend that visits to Senior Care Facilities still be prohibited. In addition to the Governor’s report, The White House Administration recommends that visits to senior care facilities or hospitals should be prohibited until Phase Three of the nation’s opening approach. Civitas Senior Living is confirming that we have learned that 1 resident at The Avenues of Park Forest, located at 3344 Forest Lane Farmers Branch TX, 75234 tested positive for the COVID-19 virus. The community is following local, state, and federal guidelines pertaining to a positive COVID-19 diagnosis in a long-term care facility. The community has implemented quarantine procedures for the entire building effective immediately, and all staff are wearing Personal Protective Equipment (PPE) in accordance with Centers for Disease Control and Prevention (CDC) guidelines. Under this order, all residents will be required to remain in their apartments for a 14-day period. All residents and staff members will continue to be screened and monitored frequently for COVID-19 symptoms. We will provide additional information as more information becomes available. The community is following local, state, and federal guidelines pertaining to a positive COVID-19 diagnosis in a long-term care facility. The community has implemented quarantine procedures for the entire building effective immediately, and all staff are wearing Personal Protective Equipment (PPE) in accordance with Centers for Disease Control and Prevention (CDC) guidelines. Under this order, all residents will be required to remain in their apartments for a 14-day period. All residents and staff members will continue to be screened and monitored frequently for COVID-19 symptoms. We will provide additional information as more information becomes available. The Texas Health and Human Services Commission has approved limited visitation rules for Assisted Living and Memory Care long-term care facilities. Our staff members at our Texas Communities have been preparing for the protocols and guidelines for the structured, scheduled visits. Several are now accepting visitor reservations and others are awaiting approval to begin allowing visitors. To schedule your visit, or to inquire about when the visitations will start at your Community, please call the Community directly and the Front Desk will assist you with making your reservation.

The Texas Health and Human Services Commission has approved limited visitation rules for Assisted Living and Memory Care long-term care facilities in Texas. Our staff members have been preparing for the protocols and guidelines for the structured, scheduled visits. We cannot start visitations in our Texas communities until we receive the Executive Order for this approval and we expect to receive that in the next couple of days. Once we receive, we will be communicating to all residents and family member contacts at our Texas communities with the guidelines of the new scheduled visitation process and will update the guidelines here.

• This week, several states have started to relax restrictions from previous stay at home orders given by the Governors of each state regarding the COVID-19 pandemic. This is a notification to inform you that even with the state’s re-opening plan, long-term care facilities are not covered in the first scheduled phases. For Texas, the Governor’s statewide Essential Services Only Order ended at the end of April and the state is preparing to begin opening select businesses at certain capacity thresholds. Because businesses will not be opened in the same ways and at the same times, he plans to continue making announcements as appropriate. In his Open Texas Report that we have attached to this letter, the Governor states that it is imperative that we continue to follow guidelines from the CDC with special information noted for the 65 and older population, which is the population that we serve through all of our communities nationwide. The Report provides information showing that the number of infections of staff and residents in nursing and long-term care facilities continues to grow and that now, more than ever, we need a consistent and strong response to effectively bend the curve of COVID-19 in these facilities. Recommendation 5 on page 10-11 of the report covers enhanced access controls to facilities and recommends “…complete limitations on any unnecessary visitations…”, still allowing visiting healthcare providers. In addition to the Governor’s report, The White House Administration recommends that visits to senior care facilities or hospitals should be prohibited until Phase Three of the nation’s opening approach. View the Guidelines here. For now, we will continue to operate under our COVID-19 pandemic response plan and follow all local, state and federal guidelines. Therefore, we are reminding you that we are still prohibiting non-essential visitors to our communities until further notice. When it is safe to do so, residents and family members will be notified through our family communications when we believe we will be allowing non-essential visitors again at our communities.

• We continue to follow guidelines from the Centers for Disease Control and Prevention, the Centers for Medicare and Medicaid Services and local health departments. In doing so, we are currently restricting all visitors and non-essential healthcare personnel to our community until further notice. Essential healthcare personnel include, but are not limited to, Physicians, Nurse Practitioners, Home Health, and Hospice Providers. There are exceptions to this for end-of-life care or other situations pursuant to the guidance of public health officials. All healthcare visitors are subject to health screenings prior to being allowed inside the community. Additionally, group activities and community events have been cancelled until further notice and our Food and Beverage department is working to deliver meals to residents in the comfort of their apartments, all steps we are taking to practice social distancing within our community. It is essential that every resident feels connected daily during this time. We have implemented resident engagement programs to keep residents’ spirits up and keep them active and engaged with each other while still practicing our social distancing. We have established daily FaceTime calls to loved ones, activities that residents can participate in either from their apartment or apartment entryway inside the hallways, and our My Special Friend program in which each Assisted Living and Memory Care resident is paired with a community staff member that can provide assistance to the resident, do activities with the resident, and answer questions for the resident. On Monday the Centers for Medicare and Medicaid Services (CMS) urged facilities that are in counties, or counties adjacent to other counties where a COVID-19 case has occurred to limit visitation to essential (medically necessary, or extenuating circumstances) visits. On Wednesday, the World Health Organization (WHO) officially declared Coronavirus a pandemic. During the President’s Coronavirus Address on 3.11.2020, he stated that “…we are strongly advising that all nursing homes for the elderly suspend all medically unnecessary visits.” Although our communities are not categorized as nursing homes, they are considered long-term care facilities for the elderly, which is the most at-risk demographic for this virus. Therefore, due to the continued pattern of transmission of COVID-19 in the United States and the evidence of community spread, our Company has made the tough decision to stop allowing non-medical visitors into our communities nationwide. We apologize for the frequent changes in the visitation process, but as you are aware, this is a rapidly evolving situation. We will assist residents in FaceTime visits with family members and other engagement activities. Physicians, Nurse Practitioners, Home Health, and Hospice Providers will still be granted access, and will undergo a travel screening, symptoms screening, and have their temperature checked prior to being allowed inside the community. If there are extenuating circumstances in which a family member needs to visit a loved one, we are asking them to please contact the Executive Director of the community.
Additionally, all community events have been cancelled for all communities nationwide for March and April.

We do not have any suspected or confirmed cases of COVID-19 in any of our communities, but feel these additional precautions are necessary to protect our residents and employees.
